Evaluation and management for attention issues and learning disabilities.
We provide evaluation and management for students with attention issues and/or learning disabilities. Psychologists and trainees in our center also offer personality assessments if recommended by a counselor.
A psycho-educational evaluation is a formal diagnostic assessment by a psychologist or under the supervision of a psychologist. The purpose of this assessment is identification of a learning disorder and/or Attention Deficit Hyperactivity Disorder (ADHD). Tests used measure cognitive functions. There is a $500 fee for an ADHD assessment and a $600 fee for the diagnostic learning assessment.
Fees are billed to your tuition statement (reduced cost available based on financial need). If a diagnosis should be made, the psychologist will determine if the nature of the learning disorder qualifies the student for reasonable accommodations under the American Disabilities Act.
If a disability is diagnosed after psycho-educational evaluation, a psychological report and comprehensive documentation will be prepared and released to Creighton University’s Office of Disability, with the student’s express written permission. This process enables the student the use of reasonable academic accommodations within the University only.

If you experience the following, you may want to consider an ADHD evaluation.
- Inattention: fails to pay close attention to detail or makes careless mistakes; difficulty sustaining attention on tasks; does not seem to listen when spoken to directly; difficulty organizing, avoids/dislikes tasks that require sustained mental effort; loses things; easily distracted by extraneous stimuli; forgetful
- Hyperactivity or impulsivity: fidgety; hard to stay seated; runs about at inappropriate times; often “on the go”; talks excessively, interrupts; difficulty waiting or sitting quietly
All students seeking medications for ADHD through the Student Health Services will be required to submit proof of prior assessments that confirm a diagnosis of ADHD. Accurate diagnosis is important because the medications used for this disorder are often controlled substances.
Assessments must be (1) made by a licensed psychiatrist, psychiatric nurse practitioner, or psychiatric physician assistant, or (2) conducted by a licensed mental health practitioner with adequate training in psychological assessment AND must include at least one valid and reliable psychometric measure of attention.
To receive medication for ADHD through the Center for Health and Counseling:
- Fax a copy of previous records with diagnostic testing to 402-280-1859.
- Previous records will be reviewed and if documentation sufficiently supports the diagnosis of ADHD, Student Counseling Services will forward a recommendation to Student Health Services. You will be notified by Student Counseling Services after the records have been reviewed.
- An examination by a Student Health Services medical provider may then be scheduled. The medical examination may include laboratory testing and an EKG.
*An ongoing appointment with a Student Health Services medical provider is required every three months for students receiving medication for ADHD from Student Health Services.
If the SCS does not approve the prior assessments or the student has NOT previously been diagnosed with ADHD:
- Schedule an appointment for a diagnostic evaluation for ADHD with Student Counseling Services (there is a $500 fee for the assessment).
- Complete the necessary forms. The packet and payments forms must be completed before your first appointment.
- Attend your appointment. If ADHD medication is recommended, Student Counseling Services will forward a recommendation to Student Health Services. You will be notified by Student Counseling Services after the records have been reviewed.
- An examination by a Student Health Services medical provider may then be scheduled. The medical examination may include laboratory testing and an EKG.

- Complete an evaluation with another provider and fax a copy of records with diagnostic testing to 402.280.1859.
