Vision Service Plan
The Vision Service Plan (VSP) provides preventive vision care benefits. Coverage for vision examinations and corrective eyewear is available.
Participating Providers
To find a participating provider, either go online to vsp.com or call VSP customer service at 800.877.7195. Creighton is a member of the VSP Choice Network. Call the provider and identify yourself as a VSP member employed by Creighton University and schedule your appointment.
Non-participating Providers
If services are received from a non-participating provider, you will pay the provider at the time of service and submit a claim to VSP for reimbursement. Keep a copy of the itemized statement for your records and send the originals to: VSP, Attn: Claims Services, P.O. Box 385018 Birmingham, AL 35238-5018.
