State Tax Exemptions

Excerpt from P-Card Policy:

This list is meant to assist University employees prior to travel.  The necessary forms or certificates are available from the Business Service Center; please plan sufficient time to request and receive the necessary forms. If a particular state is not listed below, it should be assumed that Creighton University does not have tax exempt status in that state.

    • Arizona - Tax exempt status denied
    • Arkansas - Tax exempt status denied
    • Colorado - Do not issue #'s to out-of-state supplier; Accepts our Form 13
    • Florida - 78-25-065509-57C (Exempt sales/use tax ? need letter)
    • Georgia - Tax exempt status denied
    • Illinois - E9960-3268-01 (lodging exempt after 30+ day stay)
    • Iowa - Requires state reciprocity. Nebraska laws do not allow this.
    • Kansas - Approved - (Exempt sales/use tax ? need certificate)
    • Kentucky - #OA155275 - (need certificate)
    • Massachusetts - 470-376-583 (Pending approval from State)
    • Michigan - Tax Rule 1979 AC R 205.74 (need Form 3372)
    • Minnesota - ES28808, does not apply to meals, lodging or tickets of admission
    • Mississippi - L.R.11.116, only a copy of the tax exempt letter is required.
    • Missouri - 12540382 (Exempt sales/use tax ? need letter)
    • New Jersey - 470-376-583 (need certificate)
    • New York - EX187746 (need certificate)
    • North Carolina - Tax exempt status denied
    • North Dakota ? (Exempt sales tax) ? Certificate of Exemption ND required
    • Ohio - Tax Rule 5703-9-03 (need certificate)
    • Oklahoma - Tax exempt status denied
    • Tennessee - Accepts Nebraska Form 13
    • Texas - Tax exempt status denied
    • Wisconsin - ES37325
    • Wyoming - PID#151110; Exemption form, along w/ letter from state needs to be filled out (need certificate)
